There are visual intrusive thoughts, so you could have ocd. Someone more qualified to hear your specifics would be better able to tell you
Ocd thoughts cause distress and can take any form. They can be images, words e.t.c. A fantasy is usually visual and can be both pleasant and unpleasant and has to do either with something that doesn't exist and you create or something that has to do with what you want to do in the future that hasn't happened yet. It's more about visualization. Thoughts on the other hand can be more mixed and random and usually have to do with analysis about a subject, conversations e.t.c Fantasy can be a subpart of thoughts you have. Ocd intrusive thoughts can be a fantasy too, a bad possibility of what could happen to you in the future or how you would act if you were what you fear e.t.c. I hope I didn't confuse you.
If whatever you see(in your mind) causes you intense distress, to the point you avoid anything that reminds you of it and you try to find relief by reassuring your self in any way yet it keeps on coming back, you 99% have ocd.
@LydiaK Yeah my thoughts are just visual videos of horrible things. I tried to force myself to think them and it got me worse and made me feel even more guilty and disgusted with myself. It's awful
