I had this same issue in college with French. I would get good grades on my homework. I would do great on my listening exercises, and could follow along perfectly fine in class. I would study my butt off. I would get into the test, my mind would go completely blank. I will add that I did not have test anxiety in any other class. This was a French 101 class because I never took a foreign language previous. Way back then, they weren't mandatory for graduation. That was in the late 90s. Even the professor was confused. She was like "I can tell you aren't having trouble understanding what's going on in class. You do very well on your homework and listening exercises. But what is going on with these tests? Do you study?" I told her I study all the time. I even had a friend who was a French major and offered to tutor me. I failed all of my tests. Good grades aren't everything. Colleges consider other things than just your grades. Trust me, in 5 years, all your awards won't mean anything. Am I saying you should just slack off and not do your best? But your best IS good enough. No matter how much your OCD tries to tell you otherwise. Your work ethic is much more important. I'm not trying to put you down, I'm just trying to help you see things from a different perspective. Also, your future employer won't care how many awards you earned.

Thank you so much! It's honestly the same for me; it's only this subject that I freeze up on :( but I'm still in high school so I guess I still have a lot to look forward to :)
